Social workers in Skilled Nursing Facilities (SNFs) play a vital role in nurturing residents’ psychosocial, emotional, and mental well-being. They serve as a bridge between residents, families, and our interdisciplinary care team—ensuring everyone feels supported, heard, and cared for.
What Brittanie Helps With:
· Admission & Transition: Welcoming new residents, conducting initial assessments, and helping families navigate the admission and discharge process with ease and reassurance.
· Advocacy & Resident Rights: Ensuring each resident’s dignity, choices, and personal preferences are honored and protected.
· Emotional & Psychosocial Support: Providing compassionate counseling for challenges like loneliness, depression, or anxiety, and helping residents feel at home in our community.
· Care Planning & Coordination: Partnering closely with medical and care staff to create, implement, and update individualized care plans that truly reflect each resident’s needs.
· Family Mediation: Helping residents and their loved ones communicate clearly and navigate decisions with understanding and support.
· Discharge Planning: Coordinating the services and resources needed for a safe, comfortable transition—whether returning home or moving to another level of care.
